The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay in full any outstanding dues to the college from which they wish to leave. The transfer note will be handed to the applicant, with signature. In the record register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be specified. If a notification has been filed or a disciplinary investigation has been initiated against the pharmacist at the college from which the transfer is being made, the notification and/or the disciplinary file will be forwarded to the college to which the pharmacist is transferring.
