The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the release of the transfer note, the requesting pharmacist is required to pay up to date the dues owed to the college from which they wish to leave. The transfer note will be handed over, with signature, to the applicant. In the register of pharmacists https://portal.tm-c.eu/LstFarma/ColegFarm, the college to which the pharmacist has transferred will be recorded. If, at the level of the college from which the pharmacist is transferring comparing, a complaint has been registered against them or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be sent to the college to which the pharmacist is transferring.
