1. - identity document of the applicant and all family members over the age of 14 (B.I., C.I., C.I.P.); - birth certificate of children under 14 years old; - marriage certificate – if applicable; - divorce decree/notarial act – if applicable; - affidavit stating that the persons are actually separated – if applicable; - death certificate – if applicable; - copy of final court ruling entrusting the child or approval of adoption, - decision of the Child Protection Commission regarding the measure of family foster care of the minor, order of the tutelary authority, according to the law – if applicable; - other supporting documents regarding the family composition, if applicable. The copy will be certified by handwriting regarding conformity with tenure to the original. 2. - sale-purchase contract; - sale-purchase contract with maintenance clause or with habitation clause; - rental contract; - lending contract; - inheritance certificate/succession act; - notarial power of attorney prepared by the owner of the dwelling or the holder of the rental contract, or by the legal representative of the single person who has not reached the age of 16; other documents, as appropriate. The copy will be certified by handwriting regarding conformity with the original. 3. - pension slips (for all types of pension)/ certificate from the Pension House from the month prior to the submission of the request; - unemployment indemnity payment slip and/or monthly unemployment supplement income; - state allowance slip, placement or other allowances, from the month prior to the submission of the request; - disability allowance/complementary budget slip or any other allowances granted under the law – from the month prior to the submission of the request; - incomes obtained from lease, bank interests, alienation of goods – if applicable; - affidavit of the holders/members who do not earn incomes – if applicable. The copy will be certified by handwriting regarding conformity with the original. - employment certificate for all family members, showing the net income obtained in the month prior to the submission of the request, the value of meal vouchers or mention that no vouchers are received, bonuses, meal allowances, etc. – in original. 4. - persons who own cars - copy of the car registration document (showing the year of manufacture of the car); The copy will be certified by handwriting regarding conformity with the original.*
