Annex description to the application*

Consent declaration regarding the processing of personal data*

Passport/identity card or identification document in original and copy "certified as true to the original" (with the clear signature of the official receiving the application and the round stamp)*

Special power of attorney (for submitting the waiver application) authenticated by a notary or by the Romanian diplomatic mission. The proxy will also submit a copy of their own identity document.*

Certificate regarding debts to the state issued by the local taxes and duties directorate of the city hall from the last domicile in Romania*

Declaration on own responsibility, authenticated by a notary or at the diplomatic mission of Romania, regarding debts to natural or legal persons from Romania*

Civil status documents (birth certificate, marriage certificate, certificate of name/surname change or divorce), in notarized copies. If the birth/marriage/divorce took place abroad, the certificate transcribed at the civil status office of the last domicile in Romania will be submitted, in a notarized copy. If the documents have not been transcribed, notarized copies of them will be submitted, together with the notarized translation into Romanian, with apostille or superlegalization (as applicable).*

The promise of granting citizenship of the country of domicile, in a notarized copy with apostille/legalization (if applicable) and the notarized translation into Romanian or Proof of obtaining the citizenship of the country of domicile (certificate of naturalization/certificate attesting acquisition of foreign citizenship), in a notarized copy, with apostille/legalization (if applicable) and the notarized translation into Romanian or the passport issued by foreign authorities, in a notarized/"certified true copy" with the clear signature of the official receiving the application and the round stamp*

the child's birth certificate, in a certified copy, transcribed, if applicable*

for the minor who has reached the age of 14, their authenticated consent given before the notary or the consul in the presence of a parent must also be submitted*

the parents’ affidavit stating that the minor resides abroad*

ensuring the acquisition of another citizenship which should mention the minors who will acquire the citizenship of the state of domicile or proof that they have another citizenship*

the authentic consent of the other parent, Romanian citizen (in the situation where only one of the parents requests renunciation of Romanian citizenship)*

In the case where the minor child has been entrusted by court decision to the parent who has domicile abroad and who renounces citizenship, a certified copy of the final and irrevocable court decision of entrusting the minor must be submitted.*
