The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay up to date the contribution owed to the college from which they wish to leave. The transfer note will be handed, with signature, to the applicant. In the pharmacists' record register (https://portal.net-dd.com), the college to which the pharmacist has transferred will be mentioned. If at the college from which the pharmacist is transferring there has been a complaint registered against them or a disciplinary investigation has been initiated, the complaint, respectively the disciplinary file will be sent to the college to which the pharmacist is transferring.
