In order to proceed with the issuance of a duplicate of the Certificate of Membership of the College of Pharmacists in Romania, the applicant must submit the complete documentation at the headquarters of the College of Pharmacists Bucharest, according to the provisions of Decision no. 4/2004 of the National Council of the College of Pharmacists in Romania, the documents presented below.
