The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is required to pay up to date the dues owed to the college from which they wish to leave. The transfer note will be handed, under signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint has been registered against the pharmacist at the level of the college from which the pharmacist is transferring or if a disciplinary investigation has been initiated, the complaint or the disciplinary file will be forwarded to the college to which the pharmacist is transferring.
