Issuance of medical documents for obtaining/changing the driving license*
The procedure is addressed to all applicants who wish to undergo the medical examination for obtaining/renewing their driving license, within the Specialty Outpatient Clinic of the "Dr. Ștefan Odobleja" Military Emergency Clinical Hospital, Craiova.
Reimbursement of personal contribution regarding the release of medicines for the categories of persons provided in Order M 110/2009 updated, from the territorial area assigned to the Emergency Military Clinical Hospital Dr. Ștefan Odobleja Craiova.*
The procedure applies to persons from the territorial allocation area corresponding to the Dr. Stefan Odobleja Emergency Military Clinical Hospital Craiova (SCUM Craiova) - persons from the counties of Dolj, Gorj, Mehedinți, and Olt - according to Order M110/2009 art. 4, lit. f.), h), j).
Persons who do not find all the medicines at the pharmacies contracted with SCUM Craiova can pick up their prescriptions from any community pharmacy for a fee, by requesting the following documents:
- tax invoice - in original,
- fiscal receipt - in original,
- simple prescription/ CNAS format prescription (electronic) - the "prescription" component and the "release" component - in original.
In order to recover the personal contribution paid for the medicines prescribed on C.N.A.S. type prescriptions (electronic) or simple prescriptions, the above-mentioned categories of personnel prepare a report to the commander of SC.U.M. Craiova (according to the model attached to the procedure or the form posted on the hospital's website in the "Documents" section) accompanied by the following documents:
· tax invoice - in original,
· fiscal receipt - in original,
· simple prescription - in moveable original - as applicable
· prescription "prescription" component - as applicable
· prescription "release" component - as applicable,
· copy of the identity card or ID card,
· copy of the pension slip, not older than two months.
The report and documents are submitted to the open-circuit pharmacy within the hospital by mail or in person.
The report is processed within 30 days from the registration of the request/report at the institution's registry.
