The application must be accompanied by the documents provided in art. 5 of the Accreditation Criteria for Providers of Specialized Services for Stimulating Employment, approved by Government Decision no. 277/2002, as subsequently amended and supplemented.*

The document is presented as a certified copy by the supplier and may be, as applicable: registration certificate at ONRC, authorization for carrying out independent activities, certificate of registration in the Register of associations and foundations.*

In the situation where there is an obligation to use the CAEN code, the object of activity is activities of employment placement agencies CAEN code 7810. In the case of other entities established under the law, for which there is no obligation to use the CAEN code, the object of activity can be, for example: professional information and counseling services. It is submitted in the situation where the document proving the registration of the provider does not specify the object of activity. For providers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states shall be submitted as a certified copy by the provider, accompanied by an uncertified translation into Romanian.*

Attest eligibility. Supporting documents or a declaration on the provider's own responsibility, indicating that they have not committed offenses in Romania or, as applicable, in the country of origin related to the employment services they provide. For providers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or from which it is clear that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states are submitted in a copy certified by the provider, accompanied by an uncertified translation into Romanian.*

Attest eligibility. Supporting documents or a declaration on the supplier's own responsibility attesting that it is not in Romania or, as the case may be, in the country of origin, in insolvency, in enforcement proceedings, judicial reorganization, bankruptcy, dissolution, liquidation or special administration, has not had its activities through which it provides employment services suspended or restricted, there are no final court decisions regarding convictions related to violation of legislation regarding the provision of employment services, or that it is not in other similar situations regulated by law. For suppliers from the EU and EEA who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose is accepted, or from which it clearly emerges that the requirements of the document submitted in a certified copy by the supplier accompanied by an uncertified translation in Romanian are met.*

Attests technical and managerial capacity. They are submitted in a certified copy by the supplier. For suppliers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met, is accepted. Documents issued in other EU and EEA member states are submitted in a certified copy by the supplier, accompanied by an uncertified translation in Romanian.*

Attests technical and managerial capacity. Supplier's own responsibility statement regarding the existence of the material base provided in art.3 letter B letter a)-c) of the accreditation criteria approved by Government Decision no.277/2002, with subsequent amendments and additions, including specific equipment, technical equipment, and other fixed assets that they commit to use for providing the service for which accreditation is requested. For suppliers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or from which it is clearly evident that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states are submitted in a copy certified by the supplier, accompanied by an uncertified translation into Romanian.*

Attests technical and managerial capacity. Supplier’s self-declaration containing information regarding the description of the offered service, the organization method and established objectives, target groups, assurance with specialized personnel. For suppliers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or from which it is clearly evident that the requirements of the document are fulfilled is accepted. Documents issued in other EU and EEA member states are submitted in a certified copy by the supplier, accompanied by an uncertified translation in Romanian.*

Attests the technical and managerial capacity. Attests the provision of personnel, as stipulated in art. 3 letter C of the accreditation criteria approved by Government Decision no. 277/2002, with subsequent amendments and completions. Experience in the field of the service for which accreditation is requested is attested by: European model curriculum vitae. For providers from the European Union and from the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states are submitted as a copy certified by the provider, accompanied by an uncertified translation into Romanian.*

Document attesting the payment of the accreditation fee, directly at the AJOFM/AMOFM Bucharest headquarters or by bank transfer to the AJOFM/AMOFM Bucharest account.*
