Copy of the Identity Card or Bulletin and Copy of the Unique Registration Code at the Trade Register including the authorization annexes (if applicable)*

The confirmation certificate according to Law no. 359/2004*

Copy of the Articles of Incorporation of the commercial company*

Copy of the Sale-Purchase Contract for the space, the lease contract, as applicable, and if it is a property, Land Registry Extract*

The standard affidavit regarding compliance with the operating conditions stipulated by the specific legislation in the field: sanitary, sanitary-veterinary, environmental protection, labor protection, and fire protection, as well as the specific regulations for consumer protection for the activity carried out according to the quality standards of the products and services placed on the market*

(property deed, lease contract, association or concession contract, etc.), space sketch (materializing the functional characteristics; service area, preparation-delivery area, storage area, utility annexes)*

Copy of the utilities-sanitation service contract and copy of the latest payment invoice*

In the case of carrying out constructive modifications in the activity space or in the case of new constructions for which the building permit has been obtained and for which the reception and the reception minutes at the completion of the works have been carried out*

The fee for issuing the Agreement is paid into account RO10TREZ27421360250XXXXX opened at the Moreni Treasury and is calculated as follows: 100 lei for areas < 100 sqm, 200 lei for 101-200 sqm, 300 lei for 201-500 sqm, 4000 lei for areas > 500 sqm.*
