In the case of incorrect completion due to the fault of the educational institution issuing the original diploma or in the case of presenting the original diploma damaged to such an extent that it can no longer serve its intended purpose, based on a written request from the applicant, the educational institution that issued the document shall issue a duplicate, prepared on the basis of the documents in the archive of the educational institution which indicate the academic status of the applicant.
In case of loss, destruction, or theft of the original diploma, the application for the duplicate must be accompanied by the following documents, which are archived with a retention period of 100 years at the school inspectorate, and certified copies "in conformity with the original" at the educational institution:
a) a notarial/consular statement, given by the holder of the document, which must include all identification elements, the circumstances under which the original was lost, as well as any other details confirming the disappearance of the respective diploma. The following mention is mandatory: "the original diploma was not used as collateral/guarantee at any state or private institution, either domestic or foreign, and was not knowingly transferred to another individual or legal person";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of the birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
e) written approval from the board of administration of the school inspectorate for issuing the duplicate, following submission of the documents by the educational institution.
