In order to proceed with issuing a duplicate of the Membership Certificate of the College of Pharmacists of Romania, the applicant must submit the complete documentation to the headquarters of the Argeș College of Pharmacists, according to the provisions of Decision no. 4/2004 of the National Council of the College of Pharmacists of Romania, the documents listed below.
