- duplicates of the Certificates confirming the submission of statements of own responsibility regarding the authorization of operation;

- to the legal representative of the legal entity or PFA, II, IF;
-under the condition of submitting proof of publication in a widely circulated newspaper of the announcement of loss/theft/destruction.
