The standard application is submitted or sent by mail with confirmation of receipt from OE together with the authorization file. The application will mention: the contact details of the applicant (name, surname, postal address, email address), the types of courses for which authorization is requested, the lecturers used in the professional training processes, the data of the transport manager, etc.*

The manager of the professional training center declares that, being aware of the provisions of the Criminal Code regarding false statements, certifies that the information provided in the application is true and that the documents submitted by scanned copy on the electronic medium attached to the authorization application are consistent with the originals.*

a) the document proving fulfillment of the condition provided in art. 5 of the Norms approved by ORDER MT no. 1.212/2015; b) the list including the minimum technical-teaching equipment, as provided in art. 7 para. (2) lit. c) and, where appropriate, d) of the Norms approved by ORDER MT no. 1.212/2015; c) for the types of courses for which the curriculum is not provided in a normative act, the training curriculum corresponding to the types of courses for which authorization is requested, compliant with the applicable legislation, in which the topics and the duration of the course structured by hours are highlighted; in cases where the legislation does not provide otherwise, the training courses will be scheduled so that each day of training includes a maximum of 8 hours of 60 minutes each, including breaks; d) for the manager of the training and professional development center: (i) the appointment decision showing that he/she permanently and effectively leads the activity of the training and professional development center and is mandated to represent the center in relations with the authorities; (ii) higher education diploma; (iii) documents proving the required experience in the field of road transport; (iv) employment contract concluded in accordance with the law; and (v) criminal record certificate and history of sanctions regarding the road traffic regime or, in the case of citizens of other Member States of the European Union or of the European Economic Area, a notarized statement on their own responsibility attesting to the fulfillment of the condition provided in art. 7 para. (1) lit. c) of the Norms approved by ORDER MT no. 1.212/2015; e) the list containing the road transport lecturers who will teach the courses for which authorization is requested, indicating the type of course for each lecturer; f) for road transport lecturers who will teach courses: (i) proof of certification as a lecturer in road transport in, the specialty of the type of course to be taught; (ii) employment contract concluded according to the law; and (iii) criminal record certificate and history of sanctions regarding the road traffic regime or, in the case of citizens of other Member States of the European Union or of the European Economic Area, a notarized statement on suppling, attesting to the fulfillment of the condition provided in art. 7 para. (1) lit. c) of the Norms approved by ORDER MT no. 1.212/2015; g) the course support material for each type of course for which authorization is requested, drawn up taking into account the requirements and regulations in force for the road transport field, as valid at the date of the application for the training and professional development center’s authorization. The course support must be prepared in accordance with the training programs, including those provided at lit. c), and the content must be adapted to the learning needs of the candidates and the objectives of the course; h) in the case of requesting authorization for the development of types of courses involving practical training/exam, the list containing the vehicles used in the professional training activity, as well as the documents for each vehicle, respectively: (i) the registration certificate and the identity card of each owned vehicle; and (ii) the ownership document of each vehicle.*

Additionally, for authorization for training and improvement courses for safety advisers and/or drivers for the road transport of radioactive materials belonging to class 7 of the A.D.R., course approvals are submitted in the authorization file.*
