regarding the applicant’s status and their relationship with the child/children for whom the benefit is requested, for one of the persons who has adopted the child/ to whom the child has been entrusted for adoption/ who has the child in foster care/ emergency foster care, except for the professional maternal assistant who can benefit from these rights only for their own children, as well as the person who has been appointed as guardian. In the case of P.F.A (self-employed persons): - Certificate from the Health Insurance House (C.A.S.) stating the period during which the maternity allowance (pregnancy + confinement) was paid or a declaration on one’s own responsibility that the maternity allowance was not received - Certificates from the Public Finance Administration – stating the income earned in the last 2 fiscal years prior to the child’s birth or, as the case may be, proof of income calculated for anticipated payment of tax; - Tax decisions from the Public Finance Administration for the last 2 fiscal years prior to the child’s birth stating the date of commencement of the activity, - Single Declaration, for the last 2 fiscal years prior to the child’s birth. - Decision to suspend or terminate the activity, declaration 070, from the Public Finance Administration; For students, university graduates, pupils, etc.: - CERTIFICATE ISSUED BY THE LEARNING INSTITUTION, ATTESTING THE ATTENDANCE OF FULL-TIME COURSES WITHOUT INTERRUPTION, UP TO THE DATE OF THE APPLICATION IN THE YEAR PRIOR TO THE CHILD’S BIRTH, OR THE GRADUATION DIPLOMA ACCOMPANIED BY THE TRANSCRIPT OR A CERTIFICATE ISSUED BY THE HIGHER EDUCATION INSTITUTION IF GRADUATED UNIVERSITY IN THE YEAR PRIOR TO THE CHILD’S BIRTH IF WITHIN ONE OF THE ASSIMILATED PERIODS m) is attending, without interruption, full-time courses of pre-university education, including in the "Second Chance" program, or, as the case may be, university education at bachelor's or master's level, as well as post-university education at master's level, organized according to the law, in the country or abroad, in a field recognized by the Ministry of National Education, except for situations of interruption of studies for medical reasons; n) has doctoral student status as stipulated by National Education Law no. 1/2011, with subsequent amendments and completions o) is in the period between the completion of a form of pre-university education and the commencement, in the same calendar year, of another form of pre-university, full-time, legal and uninterrupted studies; p) is in the period between graduating full-time pre-university studies, organized under the law, and the commencement of university, full-time education, in the same calendar year; q) is in the period between the completion of a form of university education, full-time studies, with or without a graduation or diploma exam, and the commencement, in the same calendar year, of another form of university education, full-time, organized according to the law, attended without interruption; r) is in the period between completing a form of university education, at the level of bachelor’s or master’s studies, or post-university education at master’s level, full-time courses, and commencing, in the same calendar year, another form of university education at bachelor’s or master’s studies, full-time courses, organized according to the law, attended without interruption; s) is in the period between completion of a form of post-university education, full-time, and commencement, in the same calendar year, of another form of post-university education, full-time, organized according to the law, attended without interruption; t) is within the 60 days period from completion of compulsory education or, as the case may be, from graduating full-time pre-university, university at bachelor’s or master’s and post-university studies at master’s level, organized according to the law, with or without a graduation exam, for the purpose of employment or, as the case may be, entering unemployment, calculated from the 1st of the month following the completion of studies - DECLARATION ON OWN RESPONSIBILITY THAT NO TAXABLE INCOME IS GENERATED If the father applies for the child-raising and care allowance, a certificate from the mother’s employer specifying the period during which the maternity allowance was paid, i.e., prenatal and postnatal leave, is also required or a declaration on her own responsibility stating that she did not benefit from the maternity allowance.*
