The procedure describes the activities carried out in order to grant free urban transport with public transport means for persons with severe disabilities, persons with serious disabilities, their personal assistants or companions according to Law no. 448 of 2006 regarding the protection and promotion of the rights of persons with disabilities, with subsequent amendments and completions.

Applications for issuing free urban transport passes with surface public transport means are to be submitted to the Social Assistance Directorate Sibiu during the period 01 - 20 of each month, following which, after checking the fulfillment of the granting conditions, the passes shall be filled out and handed over to the applicants in the last 2 working days of the month.

Application form for granting free urban transport with surface public transport The application will be completed by the person with severe or pronounced disability/the parents of the minor with severe or pronounced disability/their legal representative/personal assistant - if the application is completed by the parent of a child with severe or pronounced disability or by the legal representative of a person with severe or pronounced disability, then in the name section you will specify the name and surname of the parent/legal representative, in the surname section you will fill in with the name and surname of the person with severe or pronounced disability (child or adult), and in the CNP item you will write the personal numeric code of the person with severe or pronounced disability. -The completed application will be signed, scanned, and uploaded*
