Model form for issuing the agricultural role certificate*

Copy of the Identity Card or the Applicant's Personal Identification Bulletin*

The issuance fee for the Certificate is 14 lei and the stamp duty is 1 leu for the year 2016 and is paid at the institution's counter.*

A copy will be submitted, as applicable: sale-purchase contract, donation contract, final and irrevocable civil judgment/decision, ownership title, heir certificate, others as such.*

Copy of lease agreement, commodatum agreement, rental agreement, concession contract, animal health certificate endorsed quarterly by the veterinarian, bovine passports, bovine ear tags, movement form, equine microchip. Other supporting documents, as applicable.*

Notarial declaration given by the head of the household to confirm certain factual situations: changes in surfaces, changes in use categories, changes/registration of constructions, clarifications regarding the legal status of certain goods in the household. In case of representation, a copy of the special notarial power of attorney, as well as a copy of the ID card of the authorized natural person or a copy of the passport for the authorized natural person with domicile abroad.*

Copy of site location plan, endorsed cadastral plan, land registry excerpt, other such documents.*
