Copy of Lease Contract, loan agreement, rental contract, concession contract, health certificate for animals endorsed quarterly by the veterinary doctor, bovine passports, bovine ear tags, movement form, horse microchip. Other supporting documents, as applicable.*

Notarial declaration given by the head of the household for the confirmation of certain factual situations: changes in areas, changes in categories of use, changes/entries of constructions, clarifications regarding the legal status of certain goods in the household. In case of representation, a copy of the special notarial power of attorney, as well as a copy of the ID card of the authorized natural person or a copy of the passport for the authorized natural person with domicile abroad.*
