In case of incorrect completion due to the fault of the educational institution issuing the original diploma or in case of submitting the original diploma in such deteriorated condition that it can no longer serve its intended purpose, based on a written request from the applicant, the educational institution that issued the document will issue a duplicate, prepared on the basis of documents found in the archive of the educational institution which prove the school record of the applicant.

 In case of loss, destruction, or theft of the original diploma, the request for issuing a duplicate must be accompanied by the following documents, which are archived for 100 years at the school inspectorate, with certified copies "according to the original" held at the educational institution:

a) the notarial/consular statement given by the document holder, which includes all necessary identification elements, the circumstances under which the original document was lost, as well as other details to confirm the disappearance of the respective diploma. The following mention is mandatory: "the original diploma was not pledged/guaranteed at any state or private institution, in the country or abroad, and was not intentionally transferred to another natural or legal person";

b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;

c) notarized copy of the birth/marriage certificate;

d) two color photos, size 3 x 4 cm;
