The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is obliged to pay up to date the contribution owed to the college from which they wish to leave. The transfer note will be handed over, under signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm) the college to which the pharmacist has transferred will be mentioned. If a complaint has been registered at the college from which the pharmacist is transferring or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be transmitted to the college to which the pharmacist is transferring.
