The resumption of membership is done based on a standard application, which the applicant, the pharmacist, submits at the headquarters of the territorial College, or by electronic means, requesting the resumption of membership starting from a certain date.
In support of the requests, the documents listed below are attached.
All documents will be signed and will bear the statement "Certified true copy of the original".
