The membership certificate of the Romanian College of Physicians provided for in paragraph (1) shall be issued on the basis of the following documents: a) registration form; b) copy of the identity document; c) certified copy of the graduation diploma from the faculty; d) copy of the official qualification title(s) in the medical profession or, where applicable, the document attesting its recognition; e) proof of practicing the medical profession in the last 5 years, i.e. a certificate from the workplace, if applicable, or proof of professional competence certification; f) criminal record or certificate of professional probity issued by the member state of origin or provenance; g) type A5 health certificate, which must also include the psychiatrist’s signature; h) sworn statement regarding the non-existence of any situation of incompatibility or unworthiness as provided by law; i) copy of the marriage certificate, if applicable; j) copy of proof of payment of the registration fee.*
