- identity document of the applicant and all family members over 14 years old (B.I., C.I., C.I.P.); - birth certificate of children under 14 years old; - marriage certificate – if applicable; - divorce decree/notarial act – if applicable; - affidavit stating that the persons are de facto separated – if applicable; - death certificate – if applicable; - copy of the final court decision for child custody or adoption approval, - decision of the Child Protection Commission regarding the measure of family placement of the minor, order of the guardianship authority, according to the law – if applicable; - other supporting documents regarding the family composition, as the case may be. The copy will be certified by handwritten statement for conformity with the original.*

- sale-purchase contract; - sale-purchase contract with maintenance clause or habitation clause; - rental contract; - loan for use contract; - heir certificate/succession act; - notarial power of attorney issued by the property owner or by the holder of the rental contract, or by the legal representative of the single person who has not reached the age of 16; other documents, as applicable. The copy will be certified by hand for conformity with the original. - certificate from the Association of Owners/Tenants showing the name, surname, personal identification number of the persons registered in the property book regarding maintenance quotas; radet code; association's email address – in original.*

- pension slips (for all types of pensions) / certificate from the Pension House from the month prior to submitting the application; - unemployment benefit payment slip and/or the monthly income supplement for unemployment; - state allowance slip, placement, or other benefits, from the month prior to submitting the application; - disability indemnity slip/complementary budget or any other allowances granted according to the law – from the month prior to submitting the application; - income obtained from lease, bank interests, sale of goods – if applicable; - affidavit on own responsibility of the holders/members who do not have income – if applicable. The copy will be certified in handwriting regarding conformity with the original. - employee certificate for all family members, showing the net income obtained in the month prior to submitting the application, the value of meal vouchers, or a mention that meal vouchers, bonuses, meal allowances, etc. are not received – in original.*

- bill for thermal energy, natural gas, electric energy – as applicable; - persons who own cars - copy of the car registration document (showing the year of manufacture of the car); - for persons requesting assistance for heating the home with electric energy, contract for disconnection from thermal energy/natural gas or certificate issued by the building administration specifying that the home is heated with electric energy; any other requested documents – as applicable. The copy will be certified by hand regarding its conformity with the original.*
