The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college they wish to leave. The transfer note will be handed, under signature, to the applicant. In the pharmacists' registration register (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If the college from which the pharmacist transfers has recorded a complaint against them or a disciplinary investigation has been initiated, the complaint or disciplinary file will be forwarded to the college to which the pharmacist transfers.
