The allowance is granted based on an application and a declaration on one's own responsibility accompanied by supporting documents regarding the composition of the family, its income, and, where applicable, regarding the attendance of school courses by the dependent children. Necessary documents for the file (in copy): – application and declaration on one's own responsibility; – the applicants' identity documents; – the children's birth certificates, their identity cards (as applicable); – marriage certificate or family record book; – final decision confirming the adoption, family placement of the minor, according to perfect law; – document showing the applicant's capacity as guardian or curator; – documents indicating that a family member is attending a form of education (school certificate for all school-aged children in the family; except are school-aged children with severe or accentuated disabilities for whom proof issued by the competent authorities is provided certifying that the respective child cannot be enrolled in any form of education provided by the law); – documents showing classification as persons with severe or accentuated disabilities or degree I or II of disability, for dependents; – net income received (the month prior to submitting the application) and the supporting documents mentioned in the application form (certificates regarding net salary, pension slips, unemployment aid, state allowance coupon, etc.); In the case of consensual union (cohabitation), considered a beneficiary family, documents regarding the family composition and income for the applicant's partner will also be presented.*
