Necessary documents for the file (in copy): – application completed and signed by the legal representative of the child; – original and copy of the child’s birth certificate; – identity document of the legal representative; – documents showing the legal status of the child in relation to the legal representative and any other supporting documents, as appropriate; – if the applicant opts to receive the state allowance in a personal account, a bank statement is required, IBAN account from the bank.*

Necessary documents for the file (in copy): – standard application – identity documents of the applicants; – child's birth certificate; – marriage certificate or family booklet, as applicable; – child's disability classification certificate, if applicable; – application approved by the management of the institution for entering childcare leave; – proof issued by the employer or competent authorities regarding income earned; – order for suspension of the individual employment contract for the period of 1 year, 2 years, or 3 years in the case of a child with a disability; – in the case of persons earning taxable income from multiple sources simultaneously, childcare leave and allowance are granted based on supporting documents regarding suspension of at least one of the activities; – applicant's tax sheet (if applicable); – if the applicant chooses to receive the allowance or incentive in a personal account, a bank account statement and IBAN account from the bank are required. If a person is already receiving childcare allowance and gives birth to another child, a new file is needed for the extension of the leave. If the person on childcare leave opts for the incentive payment, a new file must be prepared and an application submitted for the granting of the incentive. Starting from March 1, 2012, at least one month of the total childcare leave period is allocated to one of the persons who did not request this right. Therefore, regardless of the situation, the parent who is on childcare leave or receiving the insertion incentive, except for cases where the applicability of art. 11 from GEO No.111/2010 is not mandatory, will lose one month of entitlement, regardless if it is an allowance or incentive, if the other parent does not exercise their own right to leave. The new regulation applies to children born after March 1, 2012, this being imposed by virtue of the principle that the responsibility of childcare falls on both parents.*
