- the form is first filled in with the husband's data, then the wife's - the forms must be filled in using CAPITAL LETTERS - write with diacritics: ă, î, â, ș, ț or other accented letters from other languages, only if they are also present in the identity documents/passport - the correct address to be entered in the declarations is the domicile address from the identity documents (do not confuse with the residence/temporary address). If you live in a village, first enter the name of the municipality/town/commune you belong to (where the town hall is), then the name of the village - the section "We have chosen the matrimonial regime" is to be filled with the chosen matrimonial regime: legal community/separation of property/conventional community. If you choose separation of property or conventional community when submitting the marriage file, you must present the matrimonial agreement concluded at the notary - in the section "After the marriage we will bear the family name" write only the family name, without your first name - in the signature area, please fill in your name and surname before marriage.
