The request of the beneficiary or the owner, as the case may be, specifically mentioning the area, the investment to be made, CF number, cadastral number and/or topographic number, signature/stamp (legal entities), accompanied by a copy of the identity document;*

In case of representation, notarial power of attorney, respectively delegation, the decision of the general meeting of associations, the decision of the sole associate, the decision of the representative of the associative form, as applicable, in original and copy, as well as a copy of the BI/CI of the authorized individual or a copy of the passport for the authorized individual with domicile abroad;*

Copy of the ownership document (for example; ownership title, sale-purchase contract, donation contract) or another valid land holding document (for example; concession contract, lease, rental, superficies, etc.) with notarial authentication of the lease; (with the exception of requests for works provided in art.7 paragraph 14 of Law no. 50/1991 on the authorization of construction works, republished);*

Owner's consent for the realization of the investment objective on the studied site, in case the land is owned by another person than the owner; administrator's consent for the realization of the investment objective on the studied site, in case the land belongs to the public/private domain of the state - in original and copy;*

Copy of the urbanism certificate (within its validity period), accompanied by the area zoning plan and the site plan, as applicable;*

Payment can be made through the following methods: - cash at the central unit - bank transfer at the banking unit - treasury ACCOUNT ANIF VASLUI RO58TREZ656501701X006407*

The land registry excerpt for information, accompanied by the cadastral plan excerpt, issued by the office of cadastre and real estate publicity no more than 30 days before the date of submission of the documentation, as applicable (except in the case where the issuance of the construction authorization is requested for the realization and rehabilitation of land improvement infrastructure in accordance with the provisions of art. 33, paragraph 2, letter a of Law 138/2004); (except for requests for the works provided for in art. 7, paragraph 14 of Law no. 50/1991 regarding the authorization of construction works, republished); (except for investment works of public utility)*

Copy of the ANIF approval for PUZ drafting (accompanied by the Local Council Decision approving the PUZ), for verifying compliance with the conditions imposed when the approval was issued (if applicable);*

Where the situation requires it (when in the Urbanism Certificate, according to Law no. 133/2012, there is a mention of agricultural intravilan, or upon request for PUD when there are IF works that may be affected), the beneficiary's file must include a Project with the technical solution for protecting the IF works, signed and stamped by a person authorized by ANIF and also, in the file there must exist according to art. 47^1 of Law no. 350/2001, republished*

Certificate of registration and certificate of status from the Trade Registry or the document based on which the activity is conducted, in copy, in the case of the beneficiary legal entity;*
