In the case of self-employed persons (P.F.A): - Certificate from the Health Insurance House (C.A.S) with the period during which maternity allowance (pregnancy + confinement) was paid or a declaration on own responsibility that they have not benefited from maternity allowance; - Certificates from the Public Finance Administration - showing the income earned in the last 2 fiscal years before the child's birth or, if applicable, proof of income calculated for the advance payment of tax; - Tax decisions from the Public Finance Administration for the last 2 fiscal years prior to the child's birth, indicating the date of the start of activity; - The Single Declaration, for the last 2 fiscal years prior to the date of the child's birth; - Decision to suspend or stop activity, declaration 070, from the Public Finance Administration. For students, graduates, pupils, etc.: - certificate issued by the educational institution stating that they attended daytime courses without interruption, until the date of the request in the year prior to the date of the child's birth, or graduation diploma accompanied by the transcript or certificate issued by the higher education institution if they graduated in the year prior to the date of the child's birth if they are in one of the assimilated periods: - attending, without interruption, daytime courses of pre-university education, including within the "Second chance" program, or, where applicable, university studies at the bachelor's or master's level, as well as postgraduate education at the master's level, organized according to the law, in the country or abroad, in a field recognized by the Ministry of National Education, except for interruption of courses due to medical reasons; - has the status of doctoral student, under the conditions provided by the National Education Law no. 1/2011, with subsequent amendments and additions; - is in the period between the completion of a form of pre-university education and the entry, in the same calendar year, into another form of pre-university education, daytime courses, organized according to the law, attended without interruption; - is in the period between graduation from daytime courses of pre-university education, organized according to the law, and starting higher education, daytime courses, in the same calendar year; - is in the period between completion of a form of higher education, daytime courses, with or without bachelor's or diploma examination, and starting, in the same calendar year, another form of higher education, daytime courses, organized according to the law, attended without interruption; - is in the period between completion of a form of higher education, at the level of bachelor's or master's studies, as well as postgraduate education at master's level, daytime courses, and starting, in the same calendar year, another form of higher education at bachelor's or master's level, daytime courses, organized according to the law, attended without interruption; - is in the period between completion of a form of postgraduate education, daytime courses, and starting, in the same calendar year, another form of postgraduate education, daytime courses, organized according to the law, attended without interruption; - is in the 60 day period from the completion of compulsory education courses, or, as the case may be, from the graduation of daytime pre-university education courses, university studies at bachelor's or master's level, and postgraduate studies at master's level, organized according to the law, with or without graduation examination, for the purpose of employment or, as the case may be, applying for unemployment, calculated from the 1st day of the month following the completion of studies. - self-declaration that you do not earn taxable income; In the case where the father applies for the child raising and care allowance, a certificate from the mother's employer is also required specifying the period during which maternity allowance was paid, i.e. prenatal and postnatal leave, or a declaration on own responsibility that maternity allowance has not been received); - certificate issued by the educational institution stating that they attended daytime courses without interruption, until the date of the request in the year prior to the date of the child's birth, or graduation diploma accompanied by the transcript or certificate issued by the higher education institution if they graduated in the year prior to the date of the child's birth if they are in one of the assimilated periods.*
