employee certificate, pension coupon (for all types of pension)/ certificate from the Pension House from the month prior to the submission of the application; - unemployment benefit coupon and/or monthly unemployment completion income; - foster care allowance coupon or other allowances, from the month prior to the submission of the application; - disability allowance/complementary budget coupon or any other allowances granted according to the law – from the month prior to the submission of the application; - income from lease, bank interest, sale of goods – if applicable; - employee certificate for all family members, showing the net income obtained in the month prior to the submission of the application, the value of meal vouchers or a mention that no meal vouchers, bonuses, meal allowances, etc. are received – in original.*
