Resumption of membership in the College of Pharmacists*
Resumption of membership is done based on an application, which the requesting pharmacist submits at the headquarters of the Prahova College, requesting the resumption of membership starting from a certain date. In support of the requests, the documents listed below are attached.
Transfer of pharmacists to another territorial college*
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college they wish to leave. The transfer note will be handed, under signature, to the applicant. In the pharmacists' registration register (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If the college from which the pharmacist transfers has recorded a complaint against them or a disciplinary investigation has been initiated, the complaint or disciplinary file will be forwarded to the college to which the pharmacist transfers.

Current Professional Certificate Request*
The methodology for the preparation and issuance of the Current Professional Certificate was established by the Decision of the National Council of C.F.R. no. 1/2007, decision published in the Official Gazette of Romania Part I no. 324 of 15.05.2007.
Below we present the main provisions of Decision no. 1/2007, as follows:
- The Current Professional Certificate will be prepared by the College of Pharmacists of Romania and issued by the territorial college where the applicant is registered;
- The validity of the Current Professional Certificate is 3 months from the date of issue;
- Within a maximum of three days from receiving the request for the issuance of the Current Professional Certificate, the territorial college will send to the College of Pharmacists of Romania the standard request form for preparing and issuing the document;
- Within a maximum of 30 days from receiving the request, C.F.R. will prepare the certificate and will send it to the territorial college;
- The Current Professional Certificate will be issued by the College of Pharmacists of Romania which will prepare it and send it to the territorial college;
- The territorial college will prepare a Register of records – with identification data of the Current Professional Certificates issued to pharmacists in the respective county – and will hand them over to applicants with signature for receipt.
Duplicate certificate of membership of the College of Pharmacists of Romania*
In order to proceed with the issuance of a duplicate of the Membership Certificate of the College of Pharmacists of Romania, the applicant must submit the complete documentation to the headquarters of the Prahova College of Pharmacists, in accordance with the provisions of Decision no. 4/2004 of the National Council of the College of Pharmacists of Romania, the documents presented below.

Registration in the College of Pharmacists of Romania*
The membership certificate is granted to pharmacists who, upon making an express request to this effect, fulfill the following conditions: hold an official qualification title in pharmacy, are not found in any of the cases of unworthiness or incompatibility provided by law, are medically fit to practice the profession of pharmacist, have not interrupted the practice of the profession for more than 5 years or have obtained their bachelor's degree in the last 5 years, have paid the registration fee (the fee is to be paid to the account of the territorial college RO91BREL0002001133430100 in the amount of 208 lei for persons who registered within 1 YEAR from graduation and 235 lei for other persons).
