The certificate of findings issued by the National Trade Register Office, in copy, reflecting subsisting information on the applicant as of the date of submission of the authorization application and which must include the economic operator’s identification information, name, registered office address, unique registration code, registration number in the trade register office, the status of the economic operator, form of organization, duration of establishment, share capital, associates/shareholders/administrators of the economic operator, the person/persons authorized to represent the economic operator, the field of activity for which the authorization is requested, branches/subunits/subsidiaries, secondary offices/work points in Romania, or, as the case may be, another supporting document containing this information, in accordance with art. 4 para. (1) letter a) of the Regulation*
