Supporting documents showing that the holder has other persons in their care and their identity documents*

- certificate from the workplace showing the net monthly income earned in the last 12 months, prior to the month in which the application is submitted (for all employed family members); - pension slip; - unemployment slip or certificate from AJOFM (as applicable); - certificate issued by the General Directorate of Public Finance of Bihor County regarding the income earned as a result of carrying out a legally established income-generating activity (civil contracts, authorized natural persons, copyright, dividends, participations or similar); - notarized declaration for adults who do not earn any income.*

Notarial declaration, given by the applicant as well as by his first-degree family members who live and manage the household together, indicating that: a. they do not own a dwelling b. they have not owned a dwelling that was transferred after January 1, 1990 c. they have not, in any way or due to their own fault, lost the right to a housing contract after January 1, 1990 d. they have not benefitted from state support through loans, subsidies or other facilities for the construction of dwellings or other real estate investments e. they do not hold, as tenants, another dwelling from the state housing stock*

a. Comodat contract, rental contract/agreement or certificate issued by the Real Estate Patrimony Directorate for persons accommodated in the "Dignitas" center or in the accommodation centers/dormitories under the administration of the Real Estate Patrimony Directorate. b. Proof of eviction from the dwelling restituted under Law no. 112/1995, Law no. 10/2001 or Law no. 501/2002, if applicable (e.g. eviction report drawn up by a judicial executor or any other document attesting that you have left the dwelling); c. Proof that the property in which the applicant and his/her family live has been restituted to the former owners.*

- For persons classified in disability/invalidity degree 1 or 2 – the certificate of classification in disability/invalidity degree; - Medical certificates regarding the health status for the holder and the members of his/her family, and in the case where the holder or a family member suffers from one of the diseases provided in Annex no. 2 of GEO no. 40/1999 which requires the allocation of an additional room, a medical certificate issued by the competent medical institution, signed by the specialist doctor and the head of that institution, will be presented.*

For veterans and war widows, beneficiaries of the provisions of the Law of gratitude towards the hero-martyrs and fighters who contributed to the victory of the Romanian Revolution of December 1989, as well as towards persons who sacrificed their lives or suffered as a result of the anti-communist worker uprising in Brașov in November 1987, no. 341/2004, with subsequent amendments and completions, and beneficiaries of the provisions of Decree-Law no. 118/1990 regarding the granting of certain rights to persons persecuted for political reasons by the dictatorship established starting March 6, 1945, as well as those deported abroad or constituted as prisoners, republished, with subsequent amendments and completions, other entitled persons or families – documents attesting the beneficiary status of these legal provisions.*
