(1) letter a) of the Regulation*

The organizational chart of the economic operator specifying the departments responsible for carrying out activities in the field for which authorization is requested, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, according to art. 4 para. (1) letter d) of the Regulation;*

Declaration on own responsibility, in original, of the legal representative of the economic operator, by which the latter undertakes responsibility for the economic operator regarding the possession of the technical-material equipment and ensuring the minimum structure of authorized personnel, at the time of concluding contracts for carrying out one or more activities permitted according to the type of authorization held and throughout the actual period of performing these activities; the declaration shall be drawn up in accordance with the model provided in Annex no. 1, according to art. 4 para. Proof of payment of the fee for the authorization of economic operators carrying out activities in the field of natural gas, established in accordance with the provisions of the order of the ANRE president regarding the approval of the tariffs and monetary contributions charged by the National Energy Regulatory Authority, for the type of authorization requested, according to art. 4 para. (1) letter g) of the Regulation;*

For the permit intended for the design of upstream supply pipelines and surface technological installations related to natural gas storage, type PPI, according to art. 5 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type PGT; one of the authorized installers type PGT must have experience in the activity of design for this type of objectives of at least 7 years and be a graduate with higher education in engineering sciences, having the role of coordinating the activity; For the permit intended for the design of natural gas transmission systems, natural gas distribution systems, closed distribution systems, installations related to the production/storage of biogas/biomethane, as well as installations for the use of natural gas operating under high pressure, type PT, according to art. 6 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type PGT; one of the authorized installers type PGT must have experience in the activity of design for this type of objectives of at least 7 years and be a graduate with higher education in engineering sciences, having the role of coordinating the activity; For the permit intended for the design of natural gas distribution systems, closed distribution systems, as well as installations related to the activity of production/storage of biogas/biomethane operating under medium, low, and very low pressure, type PDSB, according to art. 7 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type PGD; one of the authorized installers must have experience in the design activity for this type of objectives of at least 5 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities; For the permit intended for the design of natural gas utilization installations operating under medium, reduced, and low pressure, type PDIB, according to art. 8 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type PGIU; one of the authorized installers must have experience in the field of natural gas design activity for at least 5 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities; For the permit intended for the execution of upstream supply pipelines and surface technological installations related to natural gas storage, type EPI, according to art. 10 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type EGT; one of the authorized installers must have experience in the execution activity for this type of objectives of at least 5 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities; For the permit intended for the execution of transmission systems, natural gas distribution systems, closed distribution systems, installations related to the activity of production/storage of biogas/biomethane, as well as installations for the use of natural gas operating under high pressure, type ET, according to art. 11 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type EGT; one of the authorized installers must have experience in the execution activity for this type of objectives of at least 5 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities; For the permit intended for the execution of natural gas distribution systems, closed distribution systems, as well as installations related to the activity of production/storage of biogas/biomethane operating under medium, reduced, and low pressure, type EDSB, according to art. 12 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type EGD; one of the authorized installers must have experience in the execution activity for this type of objectives of at least 3 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities; For the permit intended for the execution of natural gas utilization installations operating under medium, reduced, and low pressure, type EDIB, according to art. 13 of the Regulation: document, in copy, dated and assumed by signature by the legal representative of the economic operator/authorized person in this respect, showing that the minimum personnel structure provided in art. 4 para. (1) letter g), at the time of concluding contracts for carrying out one or more activities permitted according to the type of held authorization and throughout the effective performance of these activities, includes a minimum number of 2 authorized installers, type EGIU; one of the authorized installers must have experience in the execution activity for this type of objectives of at least 3 years and be a graduate with higher education in engineering sciences, having the role of coordinating activities;*
